to answer a few of the questions you had, the glass in harrow's shoes was his attempt to repent for his sins to try and balance his scales, he mentions it to Ammit "I had hoped my penance was enough". Layla had Scarab wings in her costume because she is now the "Scarlet Scarab", Taweret had a large Scarab as part of her apparel in the underworld (across her chest). Her father used to call her little scarab and she made him a scarlet scarf with the scarab on it, and we have seen from Steven that the host can affect the costume, so i think the outfit was mostly Taweret but pushed towards the scarab design by Layla the "Dr Harrow" parts i believe were a coping mechanism to try and lesson the insanity of what was happening, even if its more confusing to Mark/Steven, it still might help their minds cope with the insanity, and the ending of that scene i believe was them realising that it wasn't reality and making the choice to leave as they no longer needed it. edit: also Oscar Issac NEEDS to get an award for this show haha
