Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a disabling illness recognized by the World Health Organization as a neurological disease. It typically follows an infection or other stressor and presents with profound fatigue not alleviated by rest, accompanied by cognitive dysfunction, sleep disturbances, and autonomic symptoms. Onset can be sudden or gradual, affecting all ages, with a female predominance and peak onset between 20–45 years.
Additional features may include orthostatic intolerance (dizziness, tachycardia on standing), widespread pain, headaches, sore throat, tender lymph nodes, and flu-like symptoms. Women are two to four times more likely to develop ME/CFS than men.
The exact cause remains unknown. Proposed mechanisms include:
Neuroimmune dysregulation: Low-grade neuroinflammation, microglial activation, altered cytokine profiles in central and peripheral compartments.
Autonomic dysfunction: Impaired regulation of heart rate, blood pressure, and cerebral perfusion leading to orthostatic intolerance.
Mitochondrial and metabolic abnormalities: Reduced ATP production, impaired glycolysis in immune cells, mitochondrial structural changes, and a shift toward a hypometabolic “cell danger response”.
Hypothalamic–pituitary–adrenal (HPA) axis alterations: Systemic hypocortisolism and dysregulated stress responses.
