Oh yeah I saw this compilation of horror short stories and there was a clerk that was doing her paperwork wrong on purpose, little things, just to see how long she could get away with it. Delaying shipments, not marking a part fir lazguns as deffective etc. She caused a lot of deaths with tiny clerical errors.
