Ummm, the overall idea is that other Patreon tiers have weekly voting, except for the first week of each month, but... during the other weeks, everyone gets to choose. Typically, as a general rule, I try to make the top 2 most voted options appear in that patch. I must say that in this last patch, besides fulfilling the highest-tier voting, and despite not having much time, I added more functionalities that nobody chose in order to expand more paths. If I had more time, it would basically mean votes from the highest-tier patrons -> 4x4 = 16 scenes per month on that theme + 4 scenes per week from the other polls, which would be a total of 12 scenes on that. Additionally, 4 scenes from the second most voted option, if I have time... so that would be 12 weekly scenes. The thing is, lately, I only have time to work on the most voted theme, and I don't have time to implement the second most voted option. This way, most people can be satisfied with each patch, and that makes me happy as I develop this project. For example, I want to work on another project to see if I can do it during slower times, and in that project, I would only have one monthly poll for everyone, resulting in one patch per month with varied content, as the main focus is still on this game(PRISON).

I have to say this Designing by patreon poll is a less than ideal approach. Over 2-3 months you may end up causing your patreon base to self-select. If a strong voting base turns up for a specific theme, that theme will consistently win and be the majority of your content while everyone else will start falling off. If you want to let patrons choose something, imo you should limit it to at most 20% of the scenes you do every month.
