Random thought... how about giving Lexi a lisp or a speech impediment, Steph had her voice messed with, so, its only right that Lexi gets some too. Either that, or give her the Ned Flanders incapable of swearing treatment.

A disability/amputee has been an idea I've had a couple times. Was an option on a poll at some point. It would be an interesting option for the story, but I'm not sure how it fits in the world. I've put in some restrictions, where if you have an terminal or chronic illness you can't trick or pay someone into taking it. Not sure where this falls in. If anything, it could be banned now, but something that a lawyer could help them unlock for a trade to prove a point and let disabled people make trades. Nothing set in stone.

Meanwhile someone works behind the scenes and does short-term trades with a bunch of people for nose, eyes, cheeks, chin, mouth, and then does a face trade with Lexi, giving her a completely new face. (When their short-term trades expire, the others get Lexi's features distributed among them, and the original guy get his face back.)
