have you had anyone say private practice is not working. i keep getting a popup on better exception

I have a question re: Private Practice - under what circumstances does a Sim have a heart attack? I have elders that are overweight, have high blood pressure and zero fitness but they don't seem to suffer as a result. I did cheat once and make one have a heart attack, to see what happens (most impressive!) but it has never cropped up in normal gaming. Thanks for great mods.
