While this is usually gets good results on the chest, it gets pretty meh on the diaper because it is either too subtle or so strong it causes clipping. UE has a (still experimental since 5.2) Chaos Flesh simulation feature that could get good results, SOTTR only has classic cloth simulation.

are the diapers static or do they have physics attached to them? i feel like that would make sense for at least the dirty ones.
