Oh yeah! Getting to season 1’s nitty-gritty! You’ll probably read this late, but the reason why Shalltear has full HP at the end was because her creator gave her a full revive item which activated when she died.

You can use world level items more than once :) It's just that there are 20 even more broken world level items which will disappear after using it and re-appear somewhere random ...... but yeah, even "regular" world level items could make changes to the game itself , just imagine what the 20 can do
