Does this video cut off before you meant it to? On your question about rest stops: I hate to take away your innocence :) but yes, highway rest stops are notorious meeting places for men to have anonymous sex. (See the movie There's Something About Mary.) And yes, the whole episode had running jokes about the whole family being on the Atkins diet. (Although we never heard about this before.) In its popularized form, it was understood to mean no carbs (no bread, pasta, potatoes) but all the meat you want. There were little conversations about this all the way through, like GOB asking Kitty if French fries or pasta salad were OK in the restaurant. And then in the hospital, Lucille declared they were all off the diet, so in the final tag we saw everyone chowing down on bread and cake -- "Let 'Em Eat Cake."
