Cutter's shown and explained pretty thoroughly that the First Silver really has no concern for the khert's laws of reality. I mean, that is its power. As a substance beyond the control of the arbitrating khert, it makes the impossible possible, facilitating the desires of whomever has possession of it. So if Anadyne makes a good skin for it at the moment, it'll fit itself inside, convinced she is exactly the manner of person who will enable it to continue on its westward march towards destruction.
There is magic at work here. But from comments read elsewhere we can now take it as given that Ana weighs a lot more than she looks like she does.
