Tension can be good, plenty of good games have it. I don't think we need it in every game all the time though, it's better to be you even if that's different. If you do add something like that, I would suggest adding it as like a difficulty setting and not as a core feature. Your games have always had a kind of silly tone and I would hate to lose that in the pursuit of being more generic.

Maybe disposition of mean/cruel/demanding vs wholesome/cute/secure instead of a game over? I usually don’t like "waning love" mechanics in ntr games myself, feels more like an insecurity than a fetish And mechanically it feels like "lose to win" games where all the content is in gameover states
