As I recall, the First Evil can appear as anybody who has died, which of course includes Buffy and Spike.

You're wondering about why Spikes chip wasn't working when he was eating people, that's because in his brain waves, he consciously wasn't trying to hurt anyone. He didn't remember when he did kill those people. The First used him and triggered him to do their bidding.
