Yes, Neo died. For context and this is not spoiling anything, but Lana Wachowski (formerly Larry Wachowski) made The Matrix 4 to process the grief of losing their parents. They had a really difficult time losing their parents and they saw Neo and Trinity as parent figures. So they brought them back to help their grieving processes. It's honestly not a great film in a number of aspects and not necessary to watch. Many people also believe it ruins the Matrix trilogy, but I'm glad it was something that was able to help Lana Wachowski process their grief. If you want to see more Matrix that actually really adds a lot to the Matrix universe, many would highly recommend watching The Animatrix.
