For anyone who is unfamiliar with ninety-five percent of modern anime, the implication here is that Nanase's character isn't from the same world as the others, and wound up "reincarnated" in another world after an incident involving stairs.
Reincarnated in quotes because the implication is she suddenly appeared there as an adult, and wasn't actually reborn, so the term is being used loosely.
I don't consider this a massive spoiler. I expect the majority of EGS readers are familiar with this concept, and will immediately know what's up. I don't assume everyone is familiar with everything, however, so I felt I should comment on it here.
It will be addressed in-comic eventually, but given my expectation of most, but not all, readers being familiar with the concept, it would feel exclusionary to me to not point it out here.
Isekai
"Isekai" basically means "another world" in Japanese, and can be used on it's own to refer to a story that involves the protagonist(s) going to another reality. It's a common enough trope in Japanese light novels, comics, and animation that isekai is considered a subgenre.
Which is NOT why Nanase's character is the one getting isekai'd.
Nanase was chosen because I love the idea of a player unfamiliar with the game they're playing compensating by playing as an isekai protagonist, and it made sense to me that Nanase wouldn't be familiar with the game.
Also, I just really like a lot of isekai concepts, and in particular the idea of reincarnation isekai (though I prefer that the character is ACTUALLY reincarnated, and doesn't just reappear in another world).
My favorite isekai is My Next Life As a Villainess.
Where's the truck?!
The more common trope for someone reincarnating in another world that gets joked about involves a truck, but there aren't trucks in the game world.
I wanted something that would remain relevant in the new world, and there remains plenty of need to ascend and descend into various places.

The My Next Life as a Villainess is a good one, I actually like a number of the ones in both the Isekai genre, and the general Villainess genre. So I'm A Spider, So What? is one that I've been reading the light novels for. I also quite enjoy the Tearmoon Empire light novels and manga (That one's a time loop/reincarnation Villainess series.) Isekai Tensei Soudouki is another of my favorites in the genre. Also, Isekai Ojisan/Uncle from another world is hilarious.

If I recall correctly, I've seen at least one anime and read at least two books where the person world-shifted in the middle of falling down stairs. The anime had prior world shifts at other times, the first being while the protagonist was reading a book. That wasn't the last world shift, as the protagonist returned to her original world at least twice after that, so it wasn't a "died" while falling down the stairs. If I'm remembering the books correctly, they had a single world shift at the beginning, and it was unclear to them what really happened. In one of them, they thought they'd fallen into a hole and the underground society they entered just happened to be in that hole, but physics worked differently and there were times that it felt like the action in the story would've definitely taken them to above ground if it had been as simple as that. The other was more clear it wasn't a simple fall, as they fell down their cellar stairs at night to land outside during the afternoon. There was no indication in either of these what happened in the original world. None of these were recent works. I saw the anime was more than a decade before I learned of the term Isekai, and the earliest of the books was from several decades before that. After reading the author comment, I think Nanase is just adding that element as a quirk from her familiarity with Isekai, not necessarily needing it to be an element from a specific work, just knowing that it could be used as such based on what other things have been used, and thinking falling down stairs is one of the more boring options. A third book I seem to recall reading had the story start with the character picking themselves up at the bottom of a staircase, feeling bruised and battered and not remembering anything. Whether it was an Isekai or not was left unclear throughout the story, though there were hints for both the option that it was Isekai and the option that the person had just fallen and, due to being aided by someone their family had been persecuting, had a radical change in factional alignment. All of this said, trying to google for these stories isn't turning anything up, so I could easily be mistaken about all of it.

Ouch. Sorry to hear it. Nobody should have to go through that. Also, the free spa day at the Elysian FIelds Resort is nice, but it is absolutely *NOT* worth it. I understand not everyone has the same experiences I have. Just saying, so you know. I mean, when you get your punches, take it, but you'd be better off not having earned the punches, IMHO. I don't think the free spa day is worth even one death.

the initial character is quite selfish and devoid of other interests besides books, but if you can get past that for a few episodes, there is actual character growth and they decide that there are other things that are important in life, and they become much more three dimensional

I'm familiar enough with Isekai (though I've not really read any), but didn't click on the "excessively careful around stairs" bit. My first thought was about Daleks... then I wondered if she was from somewhere that didn't have stairs.

I died in 2001 and wound up in this timeline as a result, except the Me I replaced also died when the truck hit him, so maybe not Isekai? Maybe I was just Dead For A Little While? Who knows, but I did wind up with a customer loyalty card from Charon's Ferry Service that gets me a free spa day at the Elysian Fields Resort every 10 times I die and there are already 4 punch marks in it.

I've read probably 40 different isekai manga so far, and didn't make the connection at all. Also I think I've only seen one so far where Truck-kun was the cause. Usually it's something like getting stabbed while protecting someone, getting sucked into a game, summoned via magical means, etc. =/ To me Truck-kun is that mothereffer in Fuuka... EDIT: And Faraway Paladin is a pretty dang good isekai manga.

Villainess is quite fun for an Isekai. May I suggest “Ascendance of a Bookworm” as another worthy sample of the genre ? It cleverly avoid most of the pitfalls of the usual Isekai while introducing an interesting world and a lovable cast of characters (the heroine came off as a bit of a brat to me initially but you get attached very easily if you persevere).

British fantasy author Niall Teasdale has a lot of fun with the isekai concept {the "a wizard did it" subversion} in his book "The Girl Who Dreamed of a DIfferent World" - a fifteen-year-old Japanese schoolgirl shopping in Tokyo suddenly finds herself in a world of magic... Recommended, as is most of Teasdales stuff - particularly the series that begins with "Ugly"...
