For now, the only way to have a custom tattoo is to send me a picture, so I can add it to the library, and it will appear in the editor next update (which some people have already done). Unfortunately, loading custom pics on the player's side will bring a full new level of complexity. Also, I don't have the manpower to moderate the uploaded images, and without moderation, it will immediately be a bunch of swastikas and und*r*ge p*rn all over the place :(
