He did a full recovery after about 8 months of rehab(two months in Saint Mungo's and six months of freedom). By the time Harry was out of first year Hogwarts, he looked like this. (That's how I roughly pictured him going to the Malfoy Gala). With the best care in the magical world, such a recovery shouldn't be that baffling.

This has got to be Sirius after some serious (no pun intended) physical rehab and recovery from Azkaban. He looks too clean and healthy for it to be immediately after getting out, and too mature to be pre-Azkaban Sirius, since he got sent there at age 21. So this looks to me like either a 30-something Sirius who never went to Azkaban at all, or a pic of him after a full recovery from the place. Still, solid work. It definitely fits the book descriptions of the guy when he's not half-dead from the dementors.
