What are the team's thoughts on bounded accuracy for combat encounters? Is it something that you've kept in mind or are you willing to allow situations where a level 1 party would be truly unable to pierce an ancient dragon's armoured hide for example?

There has been a mention about magical items that the design will be that the characters won't have too many of them, but they will grow with them, so there will be less of a need to replace an item that has sentimental value from earlier adventures. Has the design for these kind of magical items started already and are there any examples?
