Basically it is a combination of Anus gape Width & Height, Anus Pull out and Anus Rim Thickness, you can add Anus Eversion for more exaggerated effect and fi you want you can then use sculpt (smooth tool) to smooth out the rim if "bumpy" look is not you style.

Hey I was playing around with some models but I was wondering how you got the anus rim to come out like that. Not sure what custom property gives this effect.
