Garo participated in experiments to introduce the Scientific Ninja Tool created by Amado into his hands, which caused it to fail and his arms were amputated and a standard hand blaster inserted. In addition to the introduction of Karma by Jigen, Kawaki also participated in the same operation to introduce SNT into the left hand which transforms into a blade and shoot a chakra blast, and he turned out to be compatible with her, unlike Garo.

I love how you’re enjoying how Kawaki is gradually warming up to the Leaf and the family. This arc does a great job of gradually putting the pieces together for the viewer episode by episode

This was the experiment for selecting a Karma candidate. I am sure there were more experiments done, and since Garou doesn't have Karma, that was prolly a different experiment implanting their bodies with Scientific Ninja Tools, like Kawaki's arm thingy etc
