Transphobic might be a stretch imo. I think Oda just thought it would be funny for there to be this island for Sanji. Oda exaggerated their appearances to enhance the joke. It was a bit insensitive, but this was also 10 years ago too. It's also not like Oda paints them as villains, they are all allies of the straw hats and good people. Also keep in mind okama culture in Japan may have been less of a sensitive subject back then or even now than in the west. Oda also definitely has trans positive messages in his stories too, so I wouldn't say he is transphobic or anything. I get why some would dislike this though, it's understandable.

@Surya getting surgery or not has nothing to do with someone being trans

even if we assume it isn't trans representation and just use okama as the reclaimed slur for queer its still shitty queer representation, vilifies them entirely also hate to be that person but you don't "turn" someone trans, you are trans as that's just related to not identifying with your birth sex everything related to your body is just gender affirming care sincerely a Nonbinary person

The narration implies that all of the people on this island identify as women, they are also drag queens. The concepts are not mutually exclusive. The same can be said about "okama", nowhere is it implied that all okama are trans, some are and some aren't. Also Iva doesn't turn people trans with their power, they can change people's sex.

As much as I would love to get into why this is absolutely transphobic, contrary to the central themes of one piece, and doesn’t make internal sense, I’m not going to because I don’t want to risk spoiling people to do so, and I’m not really gonna complain about a creative decision that was made over 10 years ago. Though I would hope the issues with a bunch of ugly men in dresses chasing another man, with the hopes to force themselves on him in some regard, has some pretty obvious issues, whether they are meant to represent trans people or drag queens

You can already see some meaning in the places they were sent (Some more than others at this point) Nami the Navigator and weather expert was sent to an Island that studies Weather, Franky the Cyborg and inventor was sent to a technologically advanced island where the person who created Kuma lived, Sanji whos bane is women was sent to a place devoid of them.
