It more than just never meeting the Doctor, if he would have died in this episode, River would never had been born. Rory and Amy might have still gotten married and had a child, but it wouldn't have been River.

Moffat’s original plan was that River had witnessed the Doctor’s death in her first encounter with him and he uploaded his mind to the library becoming Doctor moon Moffat has confirmed that he still believes Doctor moon to de a future Doctor.

There's a detail towards the end of this that I only noticed on a previous rewatch. River knocks the Doctor unconscious, and handcuffs him to a pipe. In The Elventh Hour Amy knocks the Doctor unconscious, and handcuffs him to a radiator. Like mother, like daughter? Was Moffat hiding a spoiler in plain sight all along?
