Without any supporting evidence, my guess is that Alex's current appearance is a reflection of him beginning to 'identify' as Abby subconsciously. So his 'residual self-image', to borrow a phrase from the Matrix movies, is blending between his actual 'Alex' appearance and his 'Abby' appearance. The more time he spends as Abby, the more his self-image will identify with that appearance.
