I've read that the reason we associate monocles with snooty rich people is that to wear one, you had to get it specially fitted to your face, precisely so it wouldn't fall out all the time, and only wealthy people could afford the expense of having this done. Disclaimer: I can't remember where I read that or verify the accuracy of it.
