Nah that's worth a couple lacerations, easy I think it depends on the vibe you wanna convey: Do you want it to be more threatening like "Get to it or else" with the knife, or do you want it more teasing without?

If you do add her holding the knife you'd have to change the position of her cheek to a resting position as there is not force holding it anymore. That would also affect how he hole would look.
