I'm thinking the little brother, should have been in a booster seat and could have possibly slid out of the seat belt at impact.

This was a very HEAVY episode. Finding out your mother did that to your boyfriend and traumatized him for life, then finding out you forgot all about your little brother who died while you held his hand is crazy.
