Whatever the reason why, the series was done way before he passed. And Rei was absolutely setup to die, probably not first but before the finale, is my guess.

I thought the author was on constant hiatus because of an illness, then he died and the illustrator decided it was best to move on and focus on his own series. I did find some info saying that the manga was originally meant to only be 6 volumes with the main characters sacrificing themselves to save humanity. Then they were trying to aim to end the series on volume 21 after they ended doing more than planned, but unfortunately there's no info on how that would've ended.

Nope, the mangaka passed away and even before that he had basically already cancelled it after the tsunami that hit Japan.

As far as I'm aware, there isn't. The illustrator decided that the series should be stopped as is and instead focus on the Triage X series.
