Also, Fake Madara's surprise at Naruto mentioning "both Madaras" comes from the fact that he assumed Kabuto only used Madara's corpse to blackmail him, and that he wasn't actually gonna use it.

The reason why you don't recognize Utakata is because he wasn't in the canon material, AKA the manga, until the war, so Naruto doesn't recognize him either in the manga. The flashbacks come from a filler episode, and the scene where they talk in Naruto's subconsciousness is also anime-exclusive.
