I wonder if the dialogue means Ethan understands Malthus is trying kill him to mock Stryder for loving a human, or if there is a possible added element that Stryder is having this nightmare as well due to the transference. The last scene from the sanctuary didn't show Al in a trance, and I wouldn't be surprised if that changed once Malthus upped the ante.
